Transcript Requests
How to Request a Transcript

A transcript request must be in writing. Please include the following;

  1. Name
  2. Birthdate
  3. Years of attendance
  4. Signature

Submit by one of the following ways:

  1. Ask the students' counselor (if the student is actively attending Sequoia) for a copy of the transcript.
  2. Fax a request to (650) 306-8893, Attn: "Registrar "
  3. Mail request to:
    Sequoia High School
    Registrar
    1201 Brewster Avenue,
    Redwood City, CA 94062

To request an official transcript, which includes the school seal, add $5 per transcript and mail the request to the school. Please make checks payable to Sequoia High School.
