How to Request a TranscriptTop of Page

A transcript request must be in writing. Please include the following;

  1. Name
  2. Birthdate
  3. Years of attendance
  4. Signature

Submit by one of the following ways:

  1. Ask the students' counselor (if the student is actively attending Sequoia) for a copy of the transcript.
  2. Fax a request to 650.562.3246, Attn: "Registrar "
  3. Mail request to:
    Sequoia High School
    Registrar
    1201 Brewster Avenue,
    Redwood City, CA 94062

To request an official transcript, which includes the school seal, add $5 per transcript and mail the request to the school. Please make checks payable to Sequoia High School.

How to Request School RecordsTop of Page

A records request must be in writing. Only Sequoia H.S. information may be released. All records are destroyed THREE years after the student's graduation year.  The permanent record that remains is the transcript.

Please include the following in your request;

  1. Name
  2. Birthdate
  3. Years of attendance
  4. Be specific on what records you are requesting.
  5. Signature

Submit by one of the following ways:

  1. Fax a request to 650.562.3246, Attn: "Registrar " and pay when you pick up record.
  2. Mail request to:

    Redwood City, CA 94062

FEE:  Each page is $0.15 (fifteen cents).  Cash if coming in person or money order payable to Sequoia High School.
