How To Request a Transcript
 

Request the transcript in writing, including name, birthdate, years of attendance and signature.

Submit by one of the following:

  1. Asking the students' counselor assisant (if the student is actively attending Sequoia) for a copy of the transcript.
  2. Fax request to (650) 306-8893, attention Registrar
  3. Mail request to:
    Sequoia High School
    Registrar
    1201 Brewster Avenue,
    Redwood City, CA 94063
To request official transcript, which includes school seal, add $5 per transcript and mail request to the school. Make check payable to Sequoia High School.
