How to Get PSAT/SAT College Board Accommodations IF Submtting Through Sequoia High School

This Is What You Must Do BEFORE submitting a College Board Accommodation Request

(If the student has a IEP, the request is submitted by the case manager)

1) Read and understand the process on the College Board Submitting a Request webpage.  Since this is an outside agency, it is the family’s responsibility to read, understand and gather all required documentation.

2) Minimum requirements for all accommodation requests through Sequoia:

Depending upon disability and accommodation requests, other outside reports are sometimes required (refer to College Board documentation webpage)

After you understand the process and have gathered all the documentation you want to submit draft one email with the subject line "College Board Accommodation Request" with all of the documents attached to Leslie Reyes lereyes@seq.org

This process needs to be completed well before the College Board deadlines.

ACT

This Is What You Must Do BEFORE Submitting an ACT Accommodation Request 

ACT Accommodation Requests

The process is a little different than SAT in that you need to register for the exam FIRST and then request accommodations. 

You will be told to contact our school to submit your accommodations request. 

1) Read and understand the following: ACT Website for Requesting Accommodations

2) Minimum Requirement for All Accommodations Requests Through Sequoia

Consent Form 

Teacher Survey on Classroom Behavior (at least one) Email the teacher this form directly and then they can email is to Ms. Yeager tyeager@seq.org to protect confidentiality.

Copy of 504 (or other formal plan)

3) After you understand the process, have gathered all the documentation you want to submit, complete the following forms (cut and pasted from the actual submission site)

Accommodations List

 AFTER you gather all your documentation, send one email with the documents as pdf versions to Ms. Yeager tyeager@seq.org submit the request. 

The process needs to be started well before the deadline.

Sequoia High School is not an ACT test center.  If you are approved for school-based testing, you will need to contact ACT to find a location to test that will support your accommodations.
