How to Get PSAT/SAT College Board Accommodations IF Submtting Through Sequoia High School
This Is What You Must Do BEFORE submitting a College Board Accommodation Request
(If the student has a IEP, the request is submitted by the case manager)
1) Read and understand the process on the College Board Submitting a Request webpage. Since this is an outside agency, it is the family’s responsibility to read, understand and gather all required documentation.
2) Minimum requirements for all accommodation requests through Sequoia:
- a. Complete the Consent Form (English) or Consent Form (Spanish)
- b. Copy of 504 (or other formal plan)
- c. Teacher Survey on Classroom Behavior (at least one teacher). Email the teacher this form directly and they can submit it to Leslie Reyes lereyes@seq.org to preserve confidentiality.
- d. Complete the SSD Student Eligibility Form.
Depending upon disability and accommodation requests, other outside reports are sometimes required (refer to College Board documentation webpage)
After you understand the process and have gathered all the documentation you want to submit draft one email with the subject line "College Board Accommodation Request" with all of the documents attached to Leslie Reyes lereyes@seq.org
This process needs to be completed well before the College Board deadlines.
