Work Permit

 
Students under the age of 18 are required to have a Work Permit in order to be employed.
 
Sequoia's Work Experience teacher is Mr. Daniel Broome, dbroome@seq.org.
 
PARENT AND STUDENT  fill out their section and sign.
EMPLOYER fills out their section and sign.
 
Submit completed Work Permit application to Mr. Broome.  He will issue the Work Permit to you.  (During the Shelter-in-Place order, you may scan or take a picture of the application and email to Mr. Broome.)
 
NOTE: A work permit may not be issued in the case of truancy, low GPA, or chronic behavioral issues and that a work permit may be revoked for the same reasons.
